IMPORTANT - One form to be completed per site before Health Station installation can be confirmed.

Select Health Station Type:

Health Station (large Maxi): Logistics Form.

Health Station (portable Mini): Logistics Form.

If you are having clinician led finger prick blood tests in addition to a SiSU Health Station, you will need to complete this additional form too. Again, please complete one logistics form per site.
